Patient-Centered Medical Home: What it Means to You

The MetCare Team Works with You to Add Value to Your Health Care Experience
Patient-Centered Medical Home, or PCMH, is not a “home” in the traditional sense. Rather, it is a way of delivering 21st century health care with an old fashioned feel. By creating a team in which your primary care doctor serves as your health care “quarterback,” the PCMH strengthens the relationship among you, your family and caregivers, your physician, and other health care professionals. PCMH delivers coordinated, comprehensive care that emphasizes the health of the whole person.
The medical home is a model that has drawn national attention as a way to improve patient satisfaction and health care quality, promote prevention, and add value to the customer. The Patient Centered Primary Care Collaborative is a coalition of major employers, consumer groups, organizations representing primary care physicians, and other stakeholders who have joined to advance the patient centered medical home. The Collaborative believes that, if implemented, the patient centered medical home will improve the health of patients and the health care delivery system.
As a patient, your role is to help better manage your health by doing three things:
- Be an active team player
- Take care of your health
- Be honest with your health care team
By participating in PCMH, you enjoy benefits, such as:
- Enhanced Communication: Getting to know your doctor and your health care team.
- Preserved Coordination of Care and Continuity of Care: Having a team that works with other medical experts if needed.
- Improved Outcomes: Information technology such as electronic prescribing, disease registries, and in the near future, electronic medical records to more effectively track, trend, and monitor your health.
- Timely Access to Care: Reaching your health care team when you need them.
The American Medical Association, American Academy of Family Physicians, American Academy of Pediatrics, American College of Physicians, and the American Osteopathic Association endorse the PMCH care model.
MetCare of Florida has transformed our medical practices from a traditional care model to a Patient-Centered Medical Home model. After testing the model for one year, MetCare discovered positive results in key areas ranging from hospitalization statistics to diagnostic screening and customer satisfaction . In March 2010, the National Committee for Quality Assurance recognized MetCare as a level three National Physician Practice Connections® – Patient-Centered Medical Home™. MetCare was the first health care organization in Florida to receive the certification. The following MetCare centers are now Medical Homes:
