Patient-Centered Medical Home is not a "home" in the literal sense. It's more of a 'homebase' - a carefully-orchestrated health care team in which your primary care doctor serves as "conductor." He or she interacts with you, your family and caregivers, your specialists and other health care professionals to deliver coordinated medical care that closes the information gaps and emphasizes your health as a whole person.

"An Introduction to the Patient-Centered Medical Home" is a short online presentation that explains in simple terms how this innovative model of care works for you and your MetCare team. The video was produced by Emmi Solutions® and the Patient-Centered Primary Care Collaborative*, a coalition of more than 400 organizations – including MetCare – who have joined together to develop and advance PCMH as a way to build lost value back into the U.S. health care delivery system.
