We operate as a Patient-Centered Medical Home (PCMH) – a new model of care that enhances the relationship between you and your primary care physician, specialists and other health professionals. A PCMH is not a place but rather a better way to deliver healthcare. It's called a "Medical Home" because your care and critical medical information are coordinated and managed through one office – a "home base." By combining innovative technology, close personal attention and time-tested principles of exceptional care, we can deliver confidence and optimal outcomes – not just prescriptions and procedures.
The PCMH model has drawn national attention as a way to improve satisfaction and health care quality, promote prevention and add value to the patient experience.

We measure our performance every day
Our system for this is simple. We monitor our team closely. And listen to you, our customer, even closer. We regularly evaluate your physician's medical records to see how care is delivered. We verify compliance to policies and procedures, national health guidelines and recognized health programs. We give all parties an open invitation to provide feedback, then follow up quickly and thoroughly on any dissatisfaction noted. After all, we can’t improve your health and quality of life if we aren’t paying attention!
